BOLALARDA GASTRODUODENIT: MUAMMOGA ZAMONAVIY QARASHLAR
Abstrakt
Adabiyotlar sharhi bolalarda gastroduodenit muammosining hozirgi holatiga bag‘ishlangan. Kasallikning tarqalishi va yoshga xos klinik ko‘rinishi bo‘yicha joriy ma'lumotlar keltirilgan. Etakchi etiologik omillar ko‘rib chiqiladi, ular orasida Helicobacter pylori asosiy rol o‘ynaydi, shuningdek, noto‘g‘ri ovqatlanish, stress ta'siri va genetik moyillik. Tasniflashlar evolyutsiyasi morfologik, etiologik va klinik-funksional xususiyatlarni hisobga olgan holda tavsiflanadi va SDDS kontseptsiyasining ilmiy maqsadlar uchun ahamiyati ta'kidlanadi. Zamonaviy diagnostika algoritmlari, jumladan, bir nechta maqsadli biopsiya bilan endoskopiya, invaziv bo‘lmagan ^13C-ureaza va najas antigeni testlari, "gastropanel" va yallig‘lanishning istiqbolli belgilari tasvirlangan. Hozirgi eradikatsiya va antisekretor terapiya sxemalari (proton pompasi ingibitorlari, kaliy blokerlari, vismut o‘z ichiga olgan to‘rtlik va ketma-ket rejimlar), gastroprotektorlarning roli, probiotiklar va parhez terapiyasi ko‘rib chiqiladi. Profilaktika va reabilitatsiya tamoyillari, shu jumladan turmush tarzi va psixo-emotsional holatni tuzatish. Quyidagi hal qilinmagan muammolar ta'kidlangan: H. pylori antibiotiklarga chidamliligining o‘sishi, lPP larni uzoq muddatli qo‘llash xavfsizligi, tasdiqlangan invaziv bo‘lmagan biomarkerlarning yo‘qligi va bolalar kasalliklari faolligi shkalasi. Ko‘p markazli tadqiqotlar va endoskopik baholashni ob'ektivlashtirish uchun II algoritmlarini ishlab chiqish zarurati qayd etilgan.
