SEMIZ BOLALARDA BUYRAKNING ERTA SHIKASTLANISHI UCHUN XAVF OMILI SIFATIDA FTO GEN POLIMORFIZMI (RS9939609)
Abstrakt
Kirish. Bolalar semizligi muammosi butun dunyo bo'ylab pediatrlarning e'tiborini jalb qilishda davom etmoqda. Komorbid patologiyaning, ya'ni buyrak shikastlanishining rivojlanishi alohida ahamiyatga ega. Semizlikda buyrak yetishmovchiligining patogenezi metabolik buzilishlarni ham, genetik moyillikni ham o'z ichiga oladi. Ushbu faktlarni o'rganish bizga semiz bolalar orasida erta profilaktika va terapevtik aralashuvlar uchun yuqori xavf guruhini aniqlash imkonini beradi. Tadqiqotning maqsadi semiz bolalarda buyrak funktsional buzilishlari va FTO gen polimorfizmi o'rtasidagi bog'liqlikni o'rganish edi. Materiallar va usullar. Alimentar ekzogen-konstitutsiyaviy semizlik bilan og'rigan jami 211 bola tekshirildi. Tekshirilgan bemorlarning namunasi yog 'to'qimalarining taqsimlanish turlariga qarab guruhlarga bo'lindi: qorin bo'shlig'i tipidagi yog 'to'qimalarining taqsimlanish turiga ega bolalar guruhi; yog 'to'qimalarining taqsimlanish turiga ega bolalar guruhi. Nazorat guruhi bir xil yoshdagi va jinsdagi, normal tana vazniga ega 104 boladan iborat edi. Bizning tadqiqotlarimiz shuni ko'rsatdiki, qorin semizligi bo'lgan bolalarda AA genotipining chastotasi statistik jihatdan ahamiyatli ustunlikka ega bo'lgan (16%) semizlik turiga ega bolalar guruhiga (28,4%) nisbatan yuqoriroq edi (28,4%). Bu bizga AA genotipini qorin semirishining rivojlanishiga moyil deb tasniflash imkonini berdi. Qorin semizligi bilan og'rigan bolalarda ham, semizlikning bir xil turi bilan og'rigan bolalarda ham A/A genotipi tashuvchilarida siydik albuminining eng yuqori darajasi (77,03±6,52 mg/l va 54,11±5,17 mg/l), T/T genotipi bilan og'rigan bolalarda esa eng past ko'rsatkich (38,83±5,03 mg/l va 21,39±3,14 mg/l; p<0,001) kuzatildi. Bir xil semizlik va mutant A/A genotipi bilan og'rigan bolalarda qonda sistatin darajasining pasayishi va glomerulyar filtratsiya tezligining oshishi kuzatildi, bu esa giperfiltratsiyani, ya'ni buyraklarda semizlik bilan bog'liq patologik o'zgarishlarning boshlanishini ko'rsatadi. Qorin semizligi va A/A genotipi bilan og'rigan bolalarda, bir xil semizlik bilan og'rigan bolalardan farqli o'laroq, sistatin darajasining oshishi va glomerulyar filtratsiya tezligining pasayishi kuzatildi, bu esa semizlik bilan bog'liq glomerulopatiyani ko'rsatadi. Xulosa. Tadqiqotlar shuni ko'rsatdiki, semiz bolalarda FTO gen polimorfizmi buyrakning funktsional shikastlanishi bilan bog'liq. Qorin semizligi bilan og'rigan A/A genotipining tashuvchilari metabolik kasalliklarning rivojlanishiga, ya'ni metabolik buyrak shikastlanishiga ko'proq moyil bo'ladi, bu mikroalbuminuriyaning sezilarli darajada oshishi, sistatin darajasining anormalligi va glomerulyar filtratsiya tezligining pasayishi bilan namoyon bo'ladi.
